Handbrake. Works perfectly and is simplicity itself to use. Free. Windows, Mac and Linux. FreeBSD licence / Open Source.

Another vote for Handbrake. I use DVDFab to rip the disc onto my hard drive and convert using Handbrake. Paul

When I used to use OSX, I used Mac The Ripper. It does what it says on the tin even though it is no longer developed. It works with OSX 10.6 and earlier. It won't work with Lion unfortunately.
